Middlefield Oaks Senior Living, located in Cottage Grove, OR is seeking a dedicated and passionate Care Partner to provide high-quality care to the residents in our community.

Responsibilities of the Care Partner

• Provide assistance with activities of daily living in a manner that supports our person-centered care philosophy.
• Provides personal care services as outlined in the individual services plans such as assistance with bathing, grooming, dressing, ambulation, positioning, assist to meals, feeding, change bed linens, laundry, etc.
• Assist in daily requirements and tasks as requested.
• Ensure residents are provided privacy, respect, and dignity.
• Follows standard precautions and infection control procedures.
• Completes state required training upon hire and annually.

Required Experience

• Possess, at a minimum, a high school diploma or equivalent.
• Be at least 18 years of age.
• We welcome applicants with prior experience in caregiving, home care, or community-based care.
• Experience in Memory Care is a plus.
• Basic computer skills.
• Must be able to speak, read, and write in the English language.

Assisted Living and Memory Care Departments Are Currently Hiring:

Job Type

Full Time for both MC & AL

• Memory Care Dept. Shift 6:00 AM to 2:00 PM Days TBD.
• Assisted Living Dept. SWING Shift from 2:00 PM to 10:00 PM. 
• Assisted Living Dept. Day Shift from 6:00 AM to 2:00 PM.
