Job Summary
We are seeking compassionate and dedicated Direct Care Workers to provide high-quality support and assistance to individuals with various needs, including seniors, individuals with disabilities, and those requiring specialized care such as Alzheimer's, dementia, or developmental disabilities. This role involves working closely with clients to promote independence, safety, and well-being in a variety of settings including assisted living facilities, nursing homes, and private residences. The ideal candidate will be committed to delivering respectful and personalized care tailored to each individual's needs.

Duties

• Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting
• Provide medication administration following care plans and medical guidelines
• Support residents with memory care needs, including Alzheimer's and dementia care strategies
• Operate EMR systems for documentation of care activities and patient information
• Prepare meals in accordance with dietary restrictions and nutritional guidelines
• Monitor vital signs and recognize signs of health deterioration or emergencies
• Use assistive devices such as Hoyer lifts and perform heavy lifting when necessary to ensure safety during transfers
• Maintain a clean and safe environment through light housekeeping tasks
• Drive clients to appointments or activities as needed, ensuring safe transportation practices
• Implement behavior management techniques and conflict resolution strategies in challenging situations
• Collaborate with healthcare teams for case management and care planning
• Document all care activities accurately while respecting HIPAA regulations

Requirements

• Prior experience in assisted living, nursing homes, home care, or working with individuals with developmental disabilities is preferred but not required. We do get you your state certificates if needed
• Ability to work with people with autism, Alzheimer's/dementia, or other cognitive impairments using specialized techniques like memory care approaches
• Strong communication skills for effective social work and case management interactions
• Valid driver’s license for transportation duties; reliable vehicle preferred
• Ability to perform heavy lifting safely and assist with transfers using Hoyer lifts or similar equipment
• Compassionate attitude with a genuine desire to improve clients’ quality of life
• Ability to work independently while following established care plans and protocols
• Familiarity with EMR systems and HIPAA compliance standards is advantageous
