Key Responsibilities:

Manage a patient caseload with a focus on closing gaps in clinical and non-clinical care.
Identify and connect patients with community resources that will support their care needs.
Provide education to patients, improving their health literacy and helping them manage chronic conditions effectively.
Oversee medication management, ensuring proper reconciliation, adherence, and coordination of refills, while identifying potential medication concerns.
Help ensure timely delivery of services, including Home Health, Durable Medical Equipment (DME), Home Infusion, and other critical needs.
Address patient questions and foster an open dialogue to understand their needs fully.
Assist with managing referrals and scheduling appointments.
Additional Requirements:

Active LPN (Compact or Multi-State) license.
Proficiency in Microsoft Office Suite.
Excellent written and verbal communication skills.
Preferred Qualifications:

Experience with Epic.
Bilingual.
Additional single-state licensures.
Remote Work Requirements:

Access to high-speed Wi-Fi.
A HIPAA-compliant home office or workspace.
Physical Requirements:

Ability to sit for extended periods of time.
Clear communication skills to speak and listen attentively, often by phone, for long periods.
Capacity to process and take thorough notes during telephone conversations in real-time.
