Position Summary: We are seeking a compassionate and skilled Registered Nurse (RN) to join our team. The RN will provide case management services and oversee RN-delegated tasks to support caregivers in the community. This role offers the opportunity to coach, mentor, and lead individuals in their professional development while ensuring high-quality care standards.

Key Responsibilities:
- Provide case management services to caregivers and clients in the community
- Train on personal cares and delegated nursing tasks to caregivers, ensuring compliance with state regulations
- Conduct assessments and develop care plans that align with clients' needs
- Offer coaching and mentorship to caregivers, fostering skill development and confidence
- Collaborate with interdisciplinary teams to enhance care coordination
- Maintain accurate documentation and uphold best practices in community nursing
Minimum Education/Experience and Requirements:
Required:
- Must be 18 years old or older
- Must be authorized to work in the United States
- Associate's degree in nursing
- Current licensure as a Registered Nurse under s. 441.06, Wisconsin Statues
- Valid driver's license, reliable transportation, and current auto insurance to travel throughout Dane County
- Must be able to push, pull, and/or lift up to 35 pounds
- Work around environmental hazards such as odors, second-hand smoke, animals, cleaning supplies, adverse living conditions, etc.
- Satisfactory Results of a WI Caregiver Background Check
Preferred:
- Bachelor's in nursing
- 1-year supervisory experience in personal care services, community-based home care (non-skilled nursing) or related program
- 3 years nursing experience in personal care services, or related program
- Experience working with people with disabilities and elderly
